Cicero is effectively the only REAL Dark brotherhood member in the entire game, the others just took on their names and do more or less random assassinations, as they have no connection to Sithis or the Night Mother, which is IMO essential to being a member of the DB. So no, let him live.

If you do that, the spectral assassin tells you essentially that Sithis does not want you to kill Cicero so I spared him. Since you can pickpocket him and receive higher grade gear at the Dawnstar Sanctuary, it is really not lucrative to kill him. PsijicThief · 10/10/2013. Nothing bad happens either way, if you kill him he's gone and not a lot of people miss him. If you let him live there's a follower with some fairly decently funny things to say from time to time. He's nuts, like outright crazy, and I happen to enjoy that. If he lives you do have a great conversation with him outside the Dawnstar Sanctuary that's well worth it's crazy in gold.

So I tried it both ways. If I let him live then I had to kill many innocents. If I killed him, then he was the only casualty. Let him live.

Kill Cicero. Once you approach and attack him, the jester will jump up into fighting position. Kill Cicero by simply attacking him with whatever weapon you have. Cicero only has around 300 to 500 health points, and he only wields a knife as a weapon so it's fairly quick (and easy) to kill him.
